Overview

The Glasgow Coma Scale (GCS) module allows ICU staff to easily record and track a patient's neurological status and level of consciousness. Instead of doing manual math, you simply select the patient's eye, verbal, and motor responses, and the system will automatically calculate the total GCS score.

When Do I Use This?

  • When admitting a new patient to the Intensive Care Unit (ICU).
  • During routine nursing rounds to monitor changes in a patient's brain function.
  • Immediately after a noticeable change in the patient's neurological status or responsiveness.

How To Use

Recording a New GCS Assessment

  1. Navigate to the patient's ICU Admission record.
  2. Select the GCS section and click + New.
  3. Ensure the Reading Time accurately reflects when the assessment took place.
  4. Select the appropriate responses from the Eye, Verbal, and Motor dropdown lists.
  5. The Score will automatically calculate as soon as you make your selections.
  6. Add any relevant observations in the Note field.
  7. Click Save to add the assessment to the patient's chart.

Field Descriptions

GCS Assessment Fields

Field Name Description
ICU The specific Intensive Care Unit admission record this assessment belongs to.
Reading Time The exact date and time the GCS assessment was performed on the patient.
Eye The patient's eye-opening response (e.g., Spontaneous, To sound, To pressure, None).
Verbal The patient's verbal response (e.g., Orientated, Confused, Words, Sounds, None).
Motor The patient's motor response (e.g., Obeys commands, Localising, Normal flexion, Abnormal flexion, Extension, None).
Score The total GCS score (ranging from 3 to 15). Note: This field is read-only and calculates automatically based on your Eye, Verbal, and Motor selections.
Note Any additional details, context, or observations regarding the assessment.

EVM Setup Fields (For Administrators)

These fields are used to configure the dropdown options for Eye, Verbal, and Motor scales.

Field Name Description
Name The description of the response (e.g., "Obeys commands").
Code A short reference code for the response.
Score The point value assigned to this specific response.
Eye / Verbal / Motor Checkboxes to indicate which category this response belongs to.
Active Whether this option is currently available for staff to select.
Tip: Always double-check the Reading Time if you are logging assessments at the end of your shift. Accurate timestamps are critical for tracking neurological trends over time.
