Intensive Care Unit (ICU) & GCS Tracking
This module helps doctors and nurses manage patient records during their stay in the Intensive Care Unit (ICU). It allows you to log patient admissions and continuously track critical neurological assessments, specifically the Glasgow Coma Scale (GCS), to monitor their level of consciousness over time.
When Do I Use This?
- When a patient is transferred or admitted into the Intensive Care Unit.
- When performing routine neurological checks (hourly, daily, or per shift) to document the patient's Glasgow Coma Scale (GCS) score.
- When you need to print or review an ICU patient's ongoing assessment report for doctor rounds or shift handovers.
How To Use
Creating an ICU Admission Record
- Go to Health → ICU from the main menu.
- Click + New to register a new ICU stay.
- Use the search bar to find and select the patient (the system will automatically suggest patient names).
- Fill in the primary admission details.
- Click Save.
Recording a Glasgow Coma Scale (GCS) Assessment
Once the patient is admitted to the ICU, you can log their GCS scores under their main record.
- Open the patient's active ICU record.
- Scroll down to the GCS Details section.
- Click Add Detail or the plus icon to add a new assessment.
- Enter the scores for Eye opening, Verbal response, and Motor response based on your clinical assessment.
- Click Save to update the patient's chart.
Generating an ICU Report
- Open the patient's ICU record.
- Click Report or Print at the top of the screen.
- The system will generate a formatted summary of the patient's ICU stay and their GCS trending scores, which you can print or save as a PDF.
Field Descriptions
Main ICU Record

| Patient Name | The name of the patient admitted to the ICU. Start typing to see suggestions. |
| Admission Date | The date and time the patient was physically moved into an ICU bed. |
GCS Assessment Details
| Field Name | Description |
|---|---|
| Assessment Time | The exact date and time the neurological check was performed. |
| Eye Response (E) | Scored from 1 (No eye opening) to 4 (Opens eyes spontaneously). |
| Verbal Response (V) | Scored from 1 (No verbal response) to 5 (Oriented and converses normally). |
| Motor Response (M) | Scored from 1 (No motor response) to 6 (Obeys commands). |
Tip: Always log the GCS score immediately after assessing the patient. Prompt documentation ensures the medical team has the most up-to-date neurological trend when making critical care decisions.
