Last updated: 2026-03-29
Overview
The Dentistry Chart module is used to record clinical findings, treatments, and examination notes for a patient's teeth. It uses customizable templates so you can quickly load the exact form you need. It also features an intelligent AI assistant that automatically summarizes your detailed clinical notes into a brief clinical interpretation as soon as you save.
When Do I Use This?
- During or after a patient's dental examination to record conditions, treatments, or observations.
- When you need to review the historical clinical notes and AI summaries of a specific tooth or area.
- When creating custom, standardized charting forms (Templates) for your clinic's different procedures.
How To Use
Creating a Patient Chart
- Go to Health → Dentistry → Chart from the main menu.
- Click + New.
- Select the patient's Tooth Profile.
- Select a Template from the dropdown. (The charting area will automatically load the layout for that specific template).
- Fill out the clinical details in the Chart area and add any additional Notes.
- Click Save.
- Once saved, the system's AI will automatically review your chart and generate a clinical summary in the Interpretation field.
Creating a Charting Template
Templates allow you to standardize the forms your staff use when charting.

- Click + New.
- Enter a Name and Description for the procedure or exam type.
- Design your form in the Form text editor (or use the AI assistant to help build the layout based on your description).

Field Descriptions
Chart Fields
| Field Name | Description |
|---|---|
| Tooth Profile | The specific patient tooth or dental profile this chart belongs to. |
| Template | The pre-designed form you want to use for this chart. Selecting this automatically loads the form layout on your screen. |
| Date | The date the examination or procedure took place. |
| Note | Any general text notes or observations regarding the visit. |
| Chart | The main interactive form area where you record your specific dental findings based on the chosen template. |
| Interpretation | A brief, concise clinical summary automatically generated by the AI after you save the chart. This field is read-only. |
Template Fields
| Field Name | Description |
|---|---|
| Name | The title of the template (e.g., "Standard Checkup", "Root Canal Findings"). |
| Description | A brief explanation of what the template is used for. This helps the AI understand how to assist you when building new templates. |
| Form | The actual layout and input fields that will appear when a user selects this template. |
| Active | Only templates marked as active will appear in the dropdown list when creating a new chart. |
Tip: You do not need to write the Interpretation yourself! Focus on filling out the detailed chart and notes. As soon as you hit save, the Minerva AI reads your findings and writes the clinical summary for you automatically.
Note: If you change the Template while you are in the middle of writing a chart, it will clear the current form layout and load the new one. Make sure you select your template before entering detailed clinical data.
